Below are links to commonly requested forms. After printing out the desired form, please complete in its entirety, obtain all appropriate signatures and return to the Registrar’s Office – Goess Center.
Forms may also be faxed to 252-321-4209 or emailed to registrar@my.pittcc.edu from a PCC email address.
Important! Placement test scores are not listed on Transcripts. A separate form is needed to see these scores. Please see the list below.
